To Send the Order Form by Mail:
Step 1: Print the Order
Form, It should only take up one page
Step 2: Please Print, and write with pen, your name, address,
quantities, sizes and prices
Mark an "X" on how you want to get the Shirt(s)
Step 3: Add up the total quantities and prices

4: E-mail me,

I'm sorry but I will
not post it on the

Step 5: Double check make EVERYTHING is correct
Step 6: Fold it
up put it in an envelope (don't forget the stamp)
To E-mail me the Order Form:
Step 1: Copy the order Form into a new E-mail,
highlight it, right click copy, in the e-mail, right click paste
(or click on the e-mail address
to open a new E-mail below *note not all computers will allow that*)
Step 2: Insert your name, E-mail Address, Home Address and Zip Code,
Mark an "X"
on how you want to get the Shirt(s)
Step 3: Click into
the correct boxes and type the correct quantities, sizes and prices
(please type in a
different color, like red)
Step 4: Add up the total Qunatities and Prices
Step 5: Double check EVERYTHING, to see if it is correct
